Single Homeless Project · Kings Cross, London, United Kingdom
No one should leave hospital with nowhere safe to go. As Homeless Pathway Coordinator, you’ll be a trusted presence for patients experiencing homelessness, helping them navigate health, housing, and support systems. You’ll manage caseloads, develop discharge plans, and advocate for patients at risk of discharge without support. This is a role where strong relationships can change the course of someone’s recovery.
About the Role
You’ll spend time with patients on wards and in A&E;, listening to what matters to them, understanding the barriers they face and making sure their voice remains at the heart of every decision. You’ll manage a caseload, complete assessments, develop clear discharge and support plans and make referrals to housing, healthcare, social care, mental health and substance use services. You’ll follow up actions, keep plans moving and advocate firmly when someone is at risk of being discharged without the right accommodation or support in place.
What You Will Do
Manage a caseload and complete assessments to determine support needs.
Develop clear discharge and support plans for patients.
Make referrals to housing, healthcare, social care, mental health and substance use services.
Follow up on actions and ensure plans are implemented.
Advocate firmly for patients at risk of discharge without appropriate support.
Work closely with hospital teams, Newham Council and SHP’s wider Health Team to coordinate care.
Build links with community services and identify gaps in existing pathways.
Share learning to help the hospital respond more effectively to people experiencing homelessness.
What We Are Looking For
Experience supporting people experiencing homelessness, multiple disadvantage or significant barriers to accessing services.
Ability to build trust with people who may feel let down by services, keeping their voice and choices at the centre of your work.
Ability to manage a caseload, assess what support is needed and turn complex situations into clear, practical next steps.
Confident communicator and advocate who can bring health, housing and support services together around an individual.
Work calmly and compassionately, understand safeguarding and risk, and are keen to keep developing your knowledge through training and reflective practice.
Join a team that’s bold, compassionate and determined to do better for the people we support and for each other. You’ll work alongside colleagues with lived experience, in a space that’s trans-inclusive, disability-friendly, and actively striving to be anti-oppressive and equitable. We’re not perfect, but we’re real. We listen. We learn. And we push forward, together. Because this isn’t just a job. It’s a chance to lead with empathy, spark change, and help build a London where no one is left behind.
