About the Role
The High Intensity Use Lead will support individuals with complex needs to reduce their reliance on emergency care services. This role involves building relationships with health and social care partners, creating personalized support plans, and advocating for service users.
What You Will Do
Supporting individuals with complex needsto reduce their use of A&E;, 999, and mental health services.
Building strong relationshipswith health, social care, and emergency service partners.
Creating personalised support plansand co-producing exit strategies with service users.
Attending multi-disciplinary meetingsand advocating for service users across agencies.
